Diabetes Management Clinic

Our Diabetes Management Clinic was developed to help patients like you better manage their condition. Here, you'll work with your doctor and other certified diabetes educators to map out a plan that could lead to a significant drop in diabetes-related complications.

To be seen at the clinic, you will need a written referral from your primary care doctor. If you do not have a primary care doctor, you can call our physician referral line at 231-672-4500 or visit our Find a Physician page.
